Internal Medicine Nurse Practitioner Job with Town Square Health in Rochester, NY
The OpportunityTown Square Health is hiring a Medical Director (Nurse Practitioner (NP)) in the Rochester area to serve as both a clinical leader and a practicing provider in our Medicare-focused, value-based care environment. This is a unique opportunity to help launch and grow our first market — combining hands-on patient care with meaningful leadership responsibilities.As Medical Director, you will lead our provider team, serve as a key liaison to central clinical leadership, and play a pivotal role in building the relationships and clinical culture that will define Town Square Health's presence in the Rochester community. You'll also help develop our provider pipeline by identifying and engaging clinical talent in the local market.This is a clinic-based role with the time, team support, and resources to practice comprehensive medicine: patient panels smaller than the national average, 20–40 minute visit lengths based on patient needs, and a team-based care model with two Medical Assistants paired to each provider.Leadership ResponsibilitiesServe as the clinical leader for the Rochester center, holding overall accountability for care quality, provider performance, and clinical cultureDirectly supervise and support staff providers, serving as their primary point of escalation and professional development resourceAct as the key connector between the Rochester clinic and Town Square Health's central clinical leadership teamLead the growth of our first market — building and nurturing relationships with local health systems, specialists, community organizations, and referral partnersPartner with operations and recruiting to build a strong local provider pipeline, actively engaging clinical talent in the Rochester marketContribute to clinical protocol development and continuous quality improvement initiativesLeverage data and analytics to drive clinical quality, including Stars and HEDIS measure performance and accurate risk documentationClinical ResponsibilitiesDeliver high-quality primary care to a Medicare patient panel in a clinic-based settingCoordinate care across the team, including Medical Assistants and specialist colleaguesHelp shape and continuously improve a care model designed for better outcomes and patient experienceMaintain excellent documentation and compliance standardsDirect patient care 3.5 days per week (28 hrs)QualificationsNurse Practitioner (NP), DNP or equivalent experience strongly preferredNational certification in at least one of the following specialties:Family Nurse PractitionerAdult-Gerontology Primary Care Nurse PractitionerAdult Nurse PractitionerGerontological Nurse PractitionerMinimum 3-5 years experience as an NPMinimum 1-2 years experience as a clinical leaderActive New York State Nurse Practitioner licensure in good standing or must obtain prior to start dateApplied for DEA registration prior to hire/start dateMust meet all applicable employer and payer credentialing requirementsAbility to meet post-offer requirements, including drug screen and health assessmentCompliance with all New York State health requirementsWhat We OfferCompetitive base salary: $130,000–$160,000, plus eligibility for bonusComprehensive benefits package including medical, dental, vision, and 401(k), PTO, $3,000 CMEA leadership role at the ground floor of a fast-growing, mission-driven healthcare companyA care model designed for quality: small panels, longer visits, AI scribes, and team-based supportThe chance to help build a high-performing clinic culture and care model from the ground upMission-driven work focused on delivering a better healthcare experience for seniors
