American Medical Associates - Director of Social Services needed for a skilled nursing facility located in the Bronx, NY!!!!!!!Salary: $95K - $110K per year based on experienceResponsibilities of the Director of Social Services-Nursing Home:Supervise, coach, and develop social work staff, fostering a collaborative and motivated environment. · Oversee comprehensive psychosocial assessments, individualized care planning, and complex discharge planning · Maintain accurate documentation in accordance with regulatory standards · Ensure compliance with New York State DOH regulations, CMS guidelines, and survey readiness · Manage and resolve grievances, complaints, and family concerns · Participate in interdisciplinary team meetings and care plan conferences · Prepare written policies and report to the AdministratorQualifications:LMSW or LCSW required (New York State licensure)Bilingual (English/Spanish) requiredMinimum 2–3+ years of experience in a skilled nursing facility or long-term care settingPrior leadership or supervisory experience strongly preferredStrong knowledge of: discharge planning, care coordination, regulatory compliance and surveysExperience handling family dynamics, grievances, and interdisciplinary collaborationExcellent communication and organizational skills#2197
