- Accountable for linking communication, collaboration, and overall alignment of initiatives across the continuum including: Acute, Post Acute, Primary Care and Ambulatory Care Management Programs, Home Health, Community Resources, Ancillary Support (Palliative Care, Hospice) and other levels of care to facilitate effective patient progression and whole person care delivery with a longitudinal lens
- Align, Liaison, and Collaborate with site specific Stakeholders such as Care Management Directors, Managers, Nursing, Hospitalist, Primary Care and Specialty Providers.
- Escalate as needed, any issues related to Analytics to request and ensure tracking of client-specific data intended to understand client business trends related to Care Continuum/Throughput Services This includes Emergency Department Utilization/Optimization, Inpatient Discharge Decisions, Inpatient Mortality, Utilization of Palliative Care, Hospice, Home Health, SNF/IRF/LTAC, and Advanced Home Services including Hospital at Home.
- Support continued development and implementation of best practice Care Continuum tools and programs designed to impact cost, quality, and experience – Discharge Decision Algorithm, Informed Choice Process, Accountable Care Management, etc.
