- Acts as case manager and team lead for patients requiring home health therapy services, when nursing is either not involved in the case or physical therapy is the primary patient need.
- Provides physical therapy treatment to the patient/family/caregiver as ordered.
- Completes accurate clinical documentation to effectively support service provided within specified timeframes Provides physical therapy to pediatric, adolescent, adult and geriatric patients based on principles of growth and development and life stages.
