- Our program offers a myriad of services and support including behavioral and mental health assessment and interventions as well as medication and psychiatric support.
- Participants include the child, youth and their family; a Care Coordinator, Behavior Counselor, and Parent/Youth Partner, school personnel, other service providers, and/or other family designates.
- Care Coordinators are responsible for providing clinical assessment (i.e., diagnosis, behavioral health assessments, client plan and progress notes) through a trauma-informed lens, therapeutic based services and/or case management to a diverse population of at-risk youth, young adults, and families who may be experiencing barriers to receiving office-based mental health services.
- Clinical services are individualized which may occur in the community, school, office, or residential setting.
- Licensed (LCSW or MFT; or, in San Diego/ Alameda/San Mateo County Licensed Professional Clinical Counselors (LPCC).
