Care Coordinators are responsible for providing clinical assessment (i.e., diagnosis, behavioral health assessments, client plan and progress notes) through a trauma-informed lens, therapeutic based services and/or case management to a diverse population of at-risk youth, young adults, and families who may be experiencing barriers to receiving office-based mental health services.. Clinical services are individualized which may occur in the community, school, office, or residential setting.. Licensed (LCSW or MFT; or, in San Diego/ Alameda/San Mateo County Licensed Professional Clinical Counselors (LPCC).. Experience with or knowledge of designing, supervising, monitoring, changing, and discontinuing individualized behavioral interventions for staff delivering behavioral services (e.g., TBS).

Shifts Available Full Time - Hourly; Mon-Fri/9am-5:30 pm Salary Range In accordance with California law, the expected salary range for this California position is between $33.03 and $53.80 per hour (registered) or $36.35 and $59.20 (licensed). Some programs may consider candidates who are able to obtain registered status within first six (6) months after hire. Registered PCC intern (PCCI)* with relevant experience with target population eligible in San Diego/Alameda/San Mateo Counties (If working in a program licensed by Community Care Licensing must meet minimum training and experience requirements).
