- The CCMTP Team. Leader reports to the CCMTP Program Director and is responsible for the direct supervision of the Transition Team in order to ensure quality service delivery to members, functioning as the senior clinician or "player coach" of the team.
- The Transition Team provides client-centered, strengths-based, and trauma-informed services to Class Members with mental illness, co-occurring substance abuse disorders and physical health concerns, utilizing a harm reduction approach.
- Assist with building positive rapport and demonstrating effective communication with facility staff, Managed Care Organizations, subsidy providers, UIC-College of Nursing, and Illinois Department of Human Services to support comprehensive, quality care.
- Must, at a minimum, meet Office of Mental Health Rule 132 credential requirements for a Mental Health Professional (MHP) including a Bachelor's degree in social work, counseling, psychology or closely related field.
- Master's degree in social work, counseling, psychology preferred.
