- SSA's assess the needs and wants of individuals on caseload, develop the individual service plan (ISP), and authorize and monitor the services to ensure individuals are living a good life.
- This SSA must be fluent in American Sign Language (ASL), have an understanding of deaf culture, and will have a caseload of predominantly hearing impaired individuals.
- Completes daily Targeted Case Management (TCM) notes to document all services delivered, TCM notes are subject to subpoena, billable TCM notes are billed to the individual's Medicaid;
- May be asked to assist with hearing impaired individuals and families not assigned on caseload;
- Provides crisis assistance to ensure that needed services occur in a critical or unexpected situation (court appearances, arranging for respite, addressing behavioral or medical emergencies, on-call responsibilities, etc.)
