- SUMMARY: The Palliative Care Social Worker is responsible for conducting psycho/social assessment/evaluations.
- The Palliative Care Social Worker will collaborate with professional palliative care team to review patient care; provide casework and group work for patients and their families; identify community resources, and assist patients and their families in connecting to these resources.
