- Summary The Hospice Social Worker uses experience, knowledge, judgment and skills to provide social services to patients and families in the hospice setting.
- The Social Worker maintains patient confidentiality and promotes patient rights as supported by agency, state and federal directives.
- Identifies cultural diversity and lifestyle choices and non-judgmentally accommodates for such differences.
- The Hospice Social Worker coordinates and delivers a service program designed to meet the psycho, social, emotional needs of the patient and family, helping them to adjust to the changes related to their illness and related end-of-life care planning.
- Education: Master’s Degree in Social Work from an institution accredited by the Council on Social Work EducationExperience: A minimum of one year of experience as a social worker in a health care setting, preferably hospice, home health, or hospital setting.
