- Saint John Paul II - The Social Worker shall interview the resident and/or family within 14 before or after admission to identify any social work needs or problems and to take a social history that includes family constellation, education, occupational background, adjustment and level of functioning, interest, support systems and observations.
- The social worker shall provide individual counseling for resident and families as needs are identified, the social worker provides leadership in group activities with families or residents and keep records of attendance and outcome.
- The Social Worker shall coordinate the discharge services for residents.
- The social worker shell perform advocacy services on behalf of the residents to ensure that concrete needs are met, such as clothing, laundry, and the resident's personal needs allowance if one is maintained.
- The social worker shall keep records of contacts made and provide clinical social work to residents as needed and to families if related to issues that directly affect the resident.
