- The Enhanced Recovery After Surgery (ERAS) Care Coordinator is responsible to develop, implement and oversee the ERAS Programs for the Department of Surgery.
- Responsibilities include development and care coordination for ERAS pathways for patients during all stages of the peri-operative continuum, ensuring the pathway is followed by all disciplines.
- Duties include: ongoing ERAS education to the patient/family, care teams and community resources; regular auditing to identify and resolve clinical issues which affect patient care, clinical efficiency, and effectiveness; and making programmatic improvements through collaboration, project management and performance improvement activities.
- Assembles multidisciplinary team to develop ERAS pathways (including supporting materials: written pathway, order sets, patient and staff education, Sharepoint site, and other tools as needed) for Divisions within the Department of Surgery as new procedures are identified for standardization of care.
- Oversees existing ERAS programs by acting as a liaison to the patient care team; coordinating information and care requirements with other care providers; resolving issues/pathway variances that could affect smooth care progression; fostering peer support; providing education to the multidisciplinary team regarding the ERAS process.
