- The purpose is to advance health equity and reduce health disparities by implementing a bold strategy that promotes optimal health for people experiencing poverty and other vulnerabilities, including addressing unmet health related social needs, by building a network of National Diabetes Prevention Program (NDPP) providers and payers across the Trinity Health service area.
- Bachelor's degree in business, healthcare administration, health/medical economics, or related field or an equivalent combination of education and experience.
- Experience working with populations including but not limited to the uninsured, communities of color, non-English speaking populations, those with access barriers including disability, age, poverty, geographic disparity, literacy and other factors is preferred.
- Ability to translate and present technical information to clinical and operations staff (and vice versa).
- Must be able to adapt to frequently changing work priorities and be able to prioritize and balance the requirements of working with the System Office and Health Ministries (HMs).
