- Christian Community Health Center (CCHC) is a Federally Qualified Health Center (non-profit) organization that provides high quality affordable healthcare and related services throughout underserved communities in Chicago.
- The RN is a core member of the clinical team working in collaboration with the CMO, physicians, mid-level clinicians, administrators and others in the day-to-day operations of a community health clinic.
- This position will work one late night per week (Thursday) to meet clinic needs.
- In collaboration with the CMO and Quality, participates in the CCHC Quality Improvement process, collects and analyzes data to monitor the delivery of patient care including but not limited to: discipline specific chart audits, triage, and other quality improvement initiatives.
- Performs clinical nursing duties as needed; Triage (walk-ins and telephone); Medication administration either directly or through supervision of staff (LPNs/Mas/CNAs); Vaccine verification; Patient education on health promotion, disease intervention, medication management, self-care, etc.
