- The Registered Nurse Case Manager is responsible for providing direct patient care, coordination of overall home care services and documentation of care for agency clients in accordance with Quality Standards and under the guidance of a Clinical Supervisor.
- Ensure quality and safe delivery of home health services including (but not limited to): case managing of client caseloads, developing a plan of care based on client assessment and OASIS evaluation, developing a plan of care using the 'interdisciplinary' approach, and ensuring that all documentation meets quality standards.
- Responsible for coordination of LPNs; provide guidance and education involving client care to LPNs, work in conjunction with direct supervisor (Clinical Supervisor) and Patient Care Coordinator (PCC) to efficiently manage patients u sing a team approach while appropriately utilizing LPNs.
- Responsible for supervision/oversight of paraprofessional staff relevant to patient care; supervise home health aides in performing designated tasks related to patient care and per the care plan, and communicate appropriate information to home health aide department.
- Comply with all Nascentia Health policies & procedures and quality initiatives; maintain patient/employee confidentiality at all times, demonstrating knowledge and use of infection control, federal/state/local government rules and regulations and compliance to all agency policies/procedures.
