Responsibilities
Works with multidisciplinary team to establish full medical history within 24 hours of admission; including verifying outpatient primary care provider, obtaining recent office records, verifying current medications and working with pharmacy to complete medication reconciliation Functions as Team Liaison by proactively reaching out to family or other designated contact person to establish contact and outline anticipated hospital course including primary team, consultants etc. Performs necessary screening surveys as required by The Joint Commission Proactively communicates with patients to ensure comprehension of the care plan and communicates any questions or concerns to the medical team Works with Care Management and Social Work to anticipate discharge needs. Assists with discharge planning by ensuring primary care provider follow-up (assist in obtaining a primary care provider if none is established), coordinate follow up appointments and imaging/lab work, obtain imaging if outside healthsystem follow up is desired. Establish protocol for post discharge follow up.
Performs other duties as required or assigned.
Qualifications
<ul>
<li>Graduate from an accredited Nursing Program required.</li>
<li>Bachelor of Science Degree in Nursing (BSN) with 5 years of related experience required.</li>
<li>Demonstrated leadership skills.</li>
</ul>
Required Licensure/Certifications
<ul>
<li>Licensed Registered Nurse with NH eligibility</li>
<li>Basic Life Support (BLS) Certificate required.</li>
