- The Patient Care Coordinator Licensed Practical Nurse under the supervision of the Oncology Nurse Manager, the Patient Care Coordinator LPN coordinates the referral process for medical and radiation oncology patients, including second opinions at centers across the country, stem cell and bone marrow transplant referrals, home care services, hospice, and diagnostic testing.
- The Patient Care Coordinator is responsible for accurate, timely, and thorough review of referrals, patient advocacy for access to care, and appropriate scheduling and tracking of appointments for patients referred by BCI providers to non-Bassett specialists, clinics and treatment centers.
- The Patient Care Coordinator provides clinical information to case managers at insurance companies on behalf of the BCI patients to obtain medically necessary treatment and services.
- The Patient Care Coordinator I also serves as an effective liaison between BCI providers, non-Bassett specialists and clinics, patients, and managed care administrative staff.
- Coordinates on time delivery of cancer drugs to patients through home infusion services, specialty pharmacies, and local pharmacies as evidenced by random supervisory observation and documentation in the patient record.
