- Innovative services offered across Chicagoland and Central Illinois include community living options, novel day programs, behavioral health services, employment programs, respite for families, and foster care for children with disabilities.
- Member of the interdisciplinary team providing case management and clinical services to persons with mental illness, who may also have a secondary diagnosis of Substance Abuse or ID/DD. Clients may also present with a variety of physical challenges.
- Provide clients with referral, linkage, and follow-up services for psychiatry and others as needed
- Maintain communication with client’s medical and benefits case manager regarding client medical benefits/insurance, SNAP status/approval, spend-down, and SSI review forms, and transportation services as needed, to ensure benefits and entitlements remain active.
- Provide community support individual and/or group skill building activities as needed.
