The Licensed Practical Nurse, under the guidance and supervision of a Registered Nurse, administers routine services in caring for sub-acute, chronic and convalescent adult/geriatric residents.. Performs and is knowledgeable of vital signs, medication administration, monitoring lab values, care of resident pre/post procedure, geriatric competency, hospice care, pain mgmt., resident safety, restraints, isolation precautions/PPE, admission and discharge of patients/residents. To provide necessary nursing care for the residents of this facility in keeping with the Mission, Vision and Goals of Catholic Health Services of Long Island. Responsibilities: <ul><li>Employee aids in the orientation of new employees</li><li>Practices within the LPN scope of practice</li><li>Identifies and reports change in resident status to RN in a timely manner</li><li>Performs and is knowledgeable of vital signs, medication administration, monitoring lab values, care of resident pre/post procedure, geriatric competency, hospice care, pain mgmt., resident safety, restraints, isolation precautions/PPE, admission and discharge of patients/residents</li><li>Employee is able to provide written and verbal educational material to residents/patients/families, and documents education as per policy</li><li>Updates/implements resident plans of care appropriately</li><li>Transcribes and documents physician orders accurately using computer order entry system</li><li>Accurately reconciles medications upon admission and discharge of resident</li><li>Participates in resident feeding and oversees dining areas as assigned</li></ul> Requirements: <ul><li>Graduate of an accredited school of practical nursing.</li><li>Licensed as an LPN in New York State</li><li>Licensed as an LPN in New York State</li><li>Basic computer skills</li><li>Previous experience preferred. Knowledge of safety and infection control policies. Knowledge of computer systems and/or prior clerical experience preferred. CPR Certified</li></ul> The pay for this position ranges from $31.86 - $39.83 per hour/per year. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
