Their condition makes it challenging for participants to live in the community. The Licensed Practical Nurse will work under the supervision of the Registered Nurse and provides general nursing care to individuals in the Day Habilitation Program. The LPN will assist in coordination of the medical services of each recipient’s primary care physician and/or medical clinic.
