- The Community Service Agency (CSA) uses a wraparound model to support youth (ages 3-20) and their families to be successful in the home or community setting.
- Our Intensive Care Coordinators (ICCs) support youth and families by building new skills, helping them to access community support, and ensuring that all service providers are working together.
- ICCs collaborate with various community systems including: schools, Primary Care Providers, Department of Children and Families, courts, and other mental health agencies.
- Additional opportunities for ICCs include: training in trauma informed care models, training and utilization of the evidence based Wraparound model, potential quarterly bonuses and productivity incentives and participation in on-call phone support for a monetary stipend.
- Facilitate the development of a Care Planning Team (CPT) which will include both formal and natural supports.
