- The Intake Coordinator must have vast knowledge of social security, Medicaid, Medicare, private insurance, and other funding streams.
- In addition, the intake coordinator will serve as the central point of contact for quality assurance, written and oral communication internally and externally on all referral processes.
- This position requires frequent travel in Johnson County, Wyandotte County, and surrounding areas.
- Facilitate the ongoing eligibility of consumers for services including medical eligibility, knowledge of private insurance benefits, Social Security, Medicaid, Medicare, Kansas Brain Injury waiver, and other funding options.
- Maintaining database for consumer information, referral contacts and service outcomes, maintain list for Aging and Disability Resource Center (ADRC) referrals and contact periodically to follow up on progress with connection to needed services and disseminating new referral information to the Vice President of Outreach, Director of Rehabilitation Services (DORS), Regional Rehabilitation Directors (RRDs) and other staff, as appropriate.
