- The CANS Assessor is responsible for arranging, conducting and completing accurate and thorough assessments using the person-centered approach.
- Outreach to individuals and their caregiver/primary supports as necessary during assigned schedule
- Schedule and coordinate the assessment process with individuals, caregiver/primary support and Care Managers/providers in the home and/or community or via telehealth using a person-centered approach
- Conduct additional interviews with caregiver/primary support and utilize multiple sources including records to verify information and coding prior to finalizing the assessment
- Strong analytical skills; detail and solution oriented; ability to work independently; excellent written and oral communication and presentation skills; and ability to use spreadsheet or database software; Proficient computer skills including, but not limited to, the ability use the following on a daily basis: Web-based applications, Email, Microsoft Word
