- Schedule : Part Time, 20 hours per week plus weekend and on-call rotation
- Under the supervision of the Home Care Supervisor and working closely with the Interdisciplinary Team (IDT), the Home Care Coordinator is a COTA/L provides the technical, organizational and communication skills necessary to efficiently assess the need for, and coordinate the delivery of, home / personal care services for adults age 55+.
- The Home Care Coordinator must demonstrate the knowledge and skills necessary to plan care for and provide service to frail elders according to assigned responsibilities, standards of practice in gerontological nursing and Community LIFE standards.
- Required Education: Graduate of accredited Certified Occupational Therapist Assistant (COTA) program.
- Required Experience: At least one year of experience working with a frail elderly population, preferably in long term care, home care or community health.
