- Extended Foster Care services includes case management, placement support, teaching life skills, and supporting clients in areas such as housing stability, employment, and education to achieve a successful transition into adulthood.
- The EFC Success Coach engages youth in a coach-like way and provides strengths-based case management and interventions as needed for youth and young adults preparing for and participating in the Extended Foster Care Program.
- Meet with each youth/young adult at their place of residence a minimum of 30 minutes face to face 1 x per month (up to 3 x’s as needed).
- Paid licensure supervision for LPC, LCSW, LMFT
- Degrees that can be considered include: social work, drug and alcohol education or counseling, psychology, criminal justice, guidance counseling, or marriage and family therapy.
