- Nationally recognized as a leader in community medicine (family practice, pediatrics, internal medicine, and geriatrics), GLFHC has clinical sites throughout the service area and is the sponsoring organization for the Lawrence Family Medicine Residency program.
- The Durable Medical Equipment (DME) Coordinator "Coordinator" is primarily responsible to ensure the ordering and securing of DME and other medical supplies for the patients at GLFHC is timely, efficient, and meets payer and other regulatory guidelines.
- As a liaison between the GLFHC ordering clinician and the patient, the coordinator keeps the ICT informed of any barriers to securing DME and medical supplies for the patient and assists in addressing any issues that delay equipment delivery.
- Reviews DME requests and ensures completed and accurate documentation prior to clinician signature and contacting DME providers.
- Refers complex patients to the CHW on the ICT if the patient requires additional care coordination to ensure loops are closed.
