Under the direct supervision of the Program Manager, is responsible for coordinating the services and support available to consumers as outlined in the Lanterman Developmental Disabilities Services Act. Has no supervisory responsibilities. Please include their full name.</li></ul> ESSENTIAL DUTIES AND RESPONSIBILITIES: <ul><li>Guided by the agency's and program's mission statement and core values, under the direction of the Program Manager, assume the daily Consumer Services Coordinator responsibilities. Complete necessary paperwork, reports, etc. on a timely basis. <ul><li>Maintain contact with consumers and their families as required.</li><li>Evaluate consumer progress and plan as identified in Individual Program Plan (IPP) on an ongoing basis.</li><li>Pursuant to the State Department of Developmental Services (DDS) provide advocacy services to consumer and his family. Route on a timely basis.</li><li>Coordinate consumer assessment including nursing, medical, psychological/developmental, education or vocational and others as needed. Do follow-up as needed.</li><li>Upon request, provide complete case coordination coverage to any consumer whose Consumer Services Coordinator is absent.</li><li>Coordinate, implement and update the CDER, Annual Review and IPP process.</li><li>As assigned, serve Medicaid Waiver cases completing required documentation on a timely and thorough basis.</li></ul></li></ul> <ul><li>Complete Placement and Liaison Activities as required. <ul><li>Under the direction of the Program Manager, act as the program's/regional center's liaison to at least two community agencies, parent groups or programs: attend meetings, provide information about the program/regional center and act as a resource for that program.</li><li>Provide written reports of activities to the Program Manager at least annually.</li><li>Under direction of the Program Manager in concert with Resource Development and Transportation Program and with the Quality Assurance Program, arrange for placement of consumers in residential facilities, day programs, and/or other programs as clinically indicated and approved by the Interdisciplinary Team.</li><li>Under the direction of the Program Manager provide case coordination services to consumers in all levels of community care facilities.</li><li>Complete a facility audit (ICRC 513) and forward to the Quality Assurance program when facility problems are identified. As assigned, do an unannounced facility audit, completing the ICRC 513 and forwarding it to the Quality Assurance program.</li></ul></li></ul> The job duties and the subheadings that follow are essential and integral to the day-to-day performance of all other job duties and responsibilities. Attend training sessions as required.</li><li>As assigned under the direction of the Program Manager, act as the program sponsor for new employees. Duties include: providing training in agency philosophy, mission, values, policy and procedures; completing general duties as outlined in the sponsorship manual; completing the required checklist.</li><li>The sponsor will stay current, up-to-date on changes in day-to-day job functions and procedures.</li><li>As assigned and under the direction of the Program Manager provide guidance for volunteers/students.</li><li>Annually, lead or participate in at least one Quality Assurance evaluation or audit of a community care facility, a health care facility, or a day program.</li><li>Observe all safety rules. Comply with the Agency's Injury and Illness Prevention Plan. Notify Program Manager of any unsafe working conditions.</li><li>Successfully complete all assignments arising out of the agency's Performance Contract.</li></ul></li></ul> <ul><li>Daily, complete administrative requirements, Purchase of Service forms and Consumer File Record Documentation in accord with agency policies and procedures. Complete all requests for action including but not limited to, those related to Eligibility Review, SIR closures, and Quality Alerts on a timely basis. Complete necessary paperwork, reports, etc. on a timely basis. <ul><li>Completion of at least 95% required case-related paperwork within designated time frames.</li><li>Coordination of the purchase of services identified in IPP, including documentation of requests for services in Quarterly Reports.</li><li>Prepare and present case for Compliance Review, Eligibility Review and other administrative case reviews.</li><li>Complete Targeted Case Management (Title XIX) documentation accurately and on a timely basis, documenting all activity, including requests for services, and securing all possible units but in no event less than agency minimum set by management.</li><li>In the event part of the record is out-of-date, make arrangements to correct the deficiency.</li><li>Organize travel efficiently and effectively.</li><li>Use equipment appropriately and keep it in good repair.
