- Medical Social Worker
- Responsibilities include: Psychosocial assessment, supportive counseling and care coordination with respect to discharge planning and service coordination for adult, geriatric, pediatric, med/surg population.
- Population groups may include adult, geriatric, pediatric, neonatal, pregnant and parenting teens on inpatient medical and surgical floors and on an outpatient basis as assigned.
- Must be able to triage crises and coordinate with collateral agencies.
