- Under supervision of the Manager of Clinical Pharmacy Services Manager or Assistant Manager, as designated, assists with the development, implementation and maintenance of educational programs to influence prescribing practices of practitioners to achieve optimal effectiveness and cost efficient high quality pharmacotherapy, provide direct patient care related to drug therapy and provide drug information.
- Provides pharmaceutical care for members including prospective, concurrent and retrospective drug utilization review, screening for drug allergies, monitoring interactions and duplications, following-up with physicians as appropriate, and counseling patients.
- Develops and/or maintains a clinical pharmacy practice site, e.g., Ambulatory Medicine, anticoagulation, Growth Factor management, Pharmacy Cardiac Risk Service, Medication Therapy Management, New Member Consults, Infectious Disease, Hepatitis C clinic, Chronic Kidney Disease, Oncology and/or Hospital based clinical pharmacy services.
- Assists practitioners and administration to ensure high quality of care in specific pharmacotherapeutic areas, e.g. those related to HEDIS measures, clinic services and initiatives.
- Residency and/or fellowship in general clinical pharmacy (e.g. Pharmacy Practice, Managed Care Pharmacy Practice, Primary Care, Ambulatory Care) or appropriate specialty area depending on the scope of the service or the ability to complete one (1) prior to hire.
