- Must be a Registered Dietitian (RD) and possess and maintain a license (LD) from the Oklahoma Board of Medical Licensure and Supervision OR be registered and licensure eligible pending Board approval.
- If a Registered Dietitian (RD) is transitioning from Provisional Licensure (PLD) to a Licensed Dietitian (LD) they must have a temporary letter authorizing practice under general supervision issued by the Oklahoma State Medical Board.
- Preferred Certified Diabetes Care and Education Specialist or CDCES. Must possess and maintain cardiopulmonary resuscitation (CPR) certification.
- OTHER QUALIFICATIONS Employee must not be and will not be under sanction by the United States Department of Health and Human Services Office of the Inspector General (OIG) or by the General Services Administration (GSA) or listed on the OIG’s Cumulative Sanction Report, or the GSA’s List of Excluded Providers, or listed on the OIG’s List of Excluded Individuals/Entities (LEIE).
- Coordinates Medical Nutrition Therapy and nutrition education activities for the clinical site.
