clinical care coordinator

Primary functions are to assist the Director of Nursing with office tasks, to assist with office tasks that require a Licensed Vocational Nurse (LVN/LPT) and to “shadow” an RN doing field visits for one year.
Works in the office with the Director of Nursing and office staff to ensure that patients are cared for according to the agencies policies and procedures and Best Practices. Goes out with a registered nurse(RN) or licensed vocational nurse (LVN) for patient in home visits to “shadow” the RN for the purpose of gaining the required one year field experience
Requirements
Education: Graduate from an accredited diploma, associate or baccalaureate nursing program.
Licensure: Active, state license as a registered nurse. Current driver’s license.
Experience: Home health experience with knowledge of intake, scheduling, authorizations, OASIS, and orders.
Skills: Proof of current CPR certification. Expert in back-office tasks. Ability to communicate with others regarding home health issues. QAPI / Chart Review Experience.
Transportation: Reliable transportation and valid auto liability insurance.

Salary Range Disclaimer:
The rates listed represent the low and high end of the range for this position. Visit and reimbursement rates vary based on visit type and employment status (1099 or W2). Rates may be adjusted in the future, and employees will be notified of these changes.
