You will be reviewing the approved services client qualifies for and ensuring they are receiving. Training will be in Baton Rouge Principal Duties and Responsibilities:<ul><li> Prepare for visits by reviewing current LA Plans of Care and other data on participants in OPTS and LOCS Long Term Care Programs to applicants.</li><li>Comply with routine participant monitoring in-person visits.</li><li>Complete Monitoring Checklist in LOCS, review provider service logs, and time sheets during in-person visits</li><li>Document participant changes, outcomes, and any participant issues or complaints</li><li>Complete Elder Protective and Adult Protective forms, as needed.</li><li>Route provider issues via LOCS to the Provider Monitoring Unit within one business day</li><li> Reported change in status via LOCS to the appropriate Client Assessment Specialist within one business</li><li>Comply with complaint procedures, escalate, and refer complaints to OAAS for Protective Service Services according to OAAS policy.</li><li>Daily travel to scheduled visits</li><li style="margin-bottom: 0in;"> Participate in ongoing Conduent and Departmental</li><li style="margin-bottom: 0in;">Participate in Continuous Quality Improvement (CQI) process.</li></ul>Educational, Training, and Experience Requirements:<ul><li>Associate degree in health or human services, preferred.</li><li>Minimum of two years of health related service experience; at least one year experience working with older adults or persons with disabilities</li><li>Knowledge of healthcare, elderly and disabled adult populations preferred.</li><li style="margin-bottom: 0in;">Knowledge of Medicaid preferred.</li></ul>Specialized Knowledge and Skills Required:<ul><li>Effective verbal, written, and interpersonal skills.</li><li>Excellent analytical skills</li><li>Ability to interpret data.</li><li>Ability to work independently and autonomously.</li><li>Ability to adhere to OAAS and CSS departmental timelines.</li><li>Valid Louisiana drivers license</li><li>Current Louisiana automobile insurance</li><li>Working knowledge of and experience in:</li><li>Department of Health and Hospitals (DHH)</li><li>Office of Aging and Adult Services</li><li>Bureau of Health Services Financing (BHSF)</li><li>Medicaid</li><li>Office of Citizens with Developmental Disabilities (OCDD)</li><li>Medicaid Home and Community Based Waivers</li><li>Understanding of healthcare, especially in areas related to elderly and disabled adults.</li></ul>Pay Transparency Laws in some locations require disclosure of compensation and/or benefits-related information. In addition to base pay, this position, based on business need, may be eligible for a bonus or incentive.
