- North Shore at Home Hospice is looking for an RN Case Manager – RN License Required
- The RN Case Manager for North Shore at Home Hospice is a critical member of the leadership team and also a member of the direct care staff.
- The RN Case Manager provides direct Hospice nursing care to our patients in order to maintain the highest’s levels of comfort and quality of life for our patients.
- The RN Case Manager will work directly with other care providers to regularly assess and change course to meet the individual needs of the patient.
- Coordination of services include communicating and making assignments for visiting nurses, home health aide/homemaker, chaplain, social worker, volunteer, and ancillary services, as well as, ordering medications and Durable Medical Equipment, and supplies.
