- Case Managers provide core case management services, including comprehensive care management, care coordination and health promotion, encouraging and assisting eligible clients to enroll in the Medicaid/health insurance plan of their choice, assist with housing needs, and provide participant and family support and referrals to community and social supports as needed.
- Provide targeted case management as a member of a multidisciplinary care team to clients enrolled in NCCBHC using team-based approach to treatment and services working collaboratively with therapists, psychiatrists/PNPs, peer specialist, substance use specialists, nurses, clients and family members
- Provide core care management services, including care coordination and healthy promotion, comprehensive transitional care, and referral to community and social supports
- Train clients on relevant chronic diseases, preventive care, medication management (medication reconciliation and adherence)
- Provide care management services in a person/family-centered, strengths-based, recovery-oriented, trauma-informed, and culturally sensitive manner
