- The Case Manager provides person centered services to promote recovery and community integration of adults with serious persistent mental illnesses and co-occurring substance use disorders within Ramsey County.
- Participate in the commitment process as mandated: provide testimony in probate court, write all required court reports, monitor stayed warrants and provisional discharge agreements and assist pre-petition screening process as necessary.
- File all legally required reports with Vulnerable Adults, the Ombudsman and Child Protection.
- Assist client with housing issues and interface with HRA and PHA staff as well as landlords.
- Bachelor's degree in behavioral sciences or related field (if no degree, 30 semester credit hours in behavioral health sciences) and 2,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders - OR-
