- The Care Management Assistant (CMA) provides administrative support to the licensed members of the Care Management team through clerical assistance with care coordination and transition planning efforts.
- The CMA supports efficient movement of patients to post-acute facilities including Skilled Nursing Facilities (SNF), Long-term Acute Care Hospitals (LTACH), Inpatient Rehab Facilities (IRF) and Intermediate Care Facilities (ICF) by receiving task triggers to create referrals; transmit to post-acute care provider facilities via HIPAA secure platform and triggering facility pre-certification initiation, or initiates pre-certification as directed.
- The CMA supports department compliance to federal and state regulations by serving patient rights notifications related to Discharge Planning (Important Message from Medicare) and Utilization Management (Medicare Outpatient Observation Notice).
