- The Bereavement Counselor cooperates with the bereavement team and other hospice staff and volunteers in developing a comprehensive.
- Develop educational programs and materials for patients, families, staff, and the community on loss, grief, and coping with bereavement.
- Provide care consistent with National Hospice and Palliative Care Organization standards of practice for bereavement programs and accreditation standards
- Conduct initial and on-going assessments of grief/loss needs or review bereavement risk assessments completed by admissions team and implement appropriate level of plan of care to ensure timely and appropriate services are provided to patients/families/caregivers.
- Support, instruct and educate the patient, family, and caregiver on grief, loss and bereavement care
