- The Behavioral Health Provider (BHP) position provides direct clinical services to Primary Care patients with a focus on providing transitions of care support for patients who have been discharged from a higher level of care.
- BHP’s will also assist with care coordination for patients on the suicide high risk list and will partner with other BHP’s to support patients with complex mental health issues.
- The position also requires Qualified Mental Health Associate (QMHA), as the position is located in the Multnomah County Community Health Center, which is a covered Federally Qualified Health Center (FQHC) under the Bureau of Primary Health Care and the Centers for Medicare and Medicaid Services of the United States Department of Health and Human Services.
