- Performs psychosocial assessment and care management.
- Provides specialized therapeutic interventions, individual and/or group psychotherapy, as needed, as well as crisis counseling and intervention services, as appropriate.
- Provides complex care management & including identification and linkage to community services and resources, and/or access or transfer to a higher level of behavioral health care (e.g. partial hospitalization, inpatient hospitalization) as appropriate, and ensures patient moves along care continuum in a timely manner.
- Promotes patient compliance and adherence to treatment as well as liaises with patient providers for collateral treatment.
- Current License to practice as a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor, or Licensed Marriage and Family Therapist (LMFT) in New York State required.
