- As an Ambulatory RN Care Manager you’ll be responsible for exemplary cross-continuum care coordination, orchestrating care across the health care system and identifying opportunities to improve patient care, services and overall population health.

- You will serve as resource for staff, colleagues, physicians and advanced practice providers to positively impact CMS core measures and National Patient Safety Goals.
- Current Registered Nurse licenses in Washington State or Multistate license endorsement (MLS)
- Certification as Certified Case Manager (CCMC), Accredited Case Manager (ACMA), Care Coordination and Transition Management (AAACN), Ambulatory Care Nursing (AAACN), or similar required or obtained within 3 years of hire (Employees hired prior to 8/1/2023 will have this requirement waived)
