You’ll employ the nursing process in the delivery of patient-centered nursing care and work collaboratively with multiple disciplines and resources facilitating wellness, and transitions of care for patients in the ambulatory environment. Responsibilities <ul><li>You will ensure the patient and family experiences seamless and safe transitions of care and engage with them as an integral and essential component of the care delivery process</li><li>You will advocate for patients and families, ensuring sensitivity to cultural and ethnic practices and beliefs</li><li>You will provide patient-centered education of disease processes and wellness opportunities</li><li>You will assist patients and families toward self-management of care including goal setting for management of chronic disease, improving medication adherence, creating a plan for behavioral change and coordinating community resources via nurse visit, telephone management, or other contacts, as needed</li><li>You will collaborate with leadership and the care team to identify opportunities, and develop a plan of action to promote appropriate utilization of resources across the continuum</li><li>You will serve as resource for staff, colleagues, physicians and advanced practice providers to positively impact CMS core measures and National Patient Safety Goals. </li></ul>Requirements <ul><li>Bachelor's degree required; preferably in nursing but other disciplines may be accepted</li><li>Current Registered Nurse licenses in Washington State or Multistate license endorsement (MLS)</li><li>Master’s degree in nursing preferred</li><li>Certification as Certified Case Manager (CCMC), Accredited Case Manager (ACMA), Care Coordination and Transition Management (AAACN), Ambulatory Care Nursing (AAACN), or similar required or obtained within 3 years of hire (Employees hired prior to 8/1/2023 will have this requirement waived)</li><li>Three (3) years of recent clinical hospital/ambulatory experience</li><li>Knowledge of care continuum (inpatient, ambulatory, post-acute, etc.) preferred </li></ul>Our Values As a MultiCare employee, we'll rely on you to reflect our core values of Respect, Integrity, Stewardship, Excellence, Collaboration and Kindness. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $107,432.00 - $179,067.00 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
