Project Transition is seeking a team of Residential Advisors to provide 24 hour; one on one support to individuals.
Our programs are designed with the primary goal of graduating each and every Member into a life that is lived in the community.
Staff member will participate in a multidisciplinary treatment team and will support and implement interventions as the leader of the Team.
Person Centered IDD Services, LLC will provide support to member(s) transitioning and living interdependently in a home within the greater community.
