Our Care Managers have the fantastic opportunity to work with our clients to help them remain independent and stay at home or get back to their own homes.
Our mission: Connecticut Community Care (CCC) identifies choices and provides services to help people of all ages, abilities and incomes to live at home.
You will serve as the primary care Manager for the residents of the housing community who are on the state home and community-based services that CCC manages.
Your job will be to assist individuals and coordinate services in order for them to remain independent and stay at home or to return home!
