- Provide the administrative oversight for clinical services within the Primary Care and Specialty Care Clinics; Oversee the coordination of case management in support of the Patient Centered Medical Home (PCMH); Provide patient and family focused care, and direct assigned employees while demonstrating leadership through direction, motivation, training, assistance, performance evaluation and corrective action; Provide collaborative leadership across inter-disciplinary teams.
- Review and revise policies and procedures and coordinate with the Chief Nursing Officer to monitor the nursing care; Coordinate with other disciplines to integrate treatment programs
- Provide guidance and direction to primary care staff and case managers, including making assessments for PCMH team-based care and assigning case managers and other staff to appropriate teams as they promote patient independence, establish patient care goals and teach patients and their families to understand conditions, medications, and self-care skills
- Oversee the networking between primary care staff, case managers, providers and agencies to provide ongoing follow up and case management services.
- i.e., HRSA reporting, FQHC, board reporting, and population health for care management.
