- Harbor provides medical, behavioral health, dental, and support services to more than 34,000 patients in Boston, the South Shore, and Cape Cod. Harbor Health also operates two Programs for All-Inclusive Care for the Elderly in Mattapan and Brockton, providing comprehensive healthcare, transportation and social services for more than 450 frail elders who continue to live with dignity and independence in the community and a Woman, Infants and Children (WIC) Nutrition Program.
- To help further the mission of Harbor, we are looking for an extremely talented Client Services Case Manager to join our Client Services/Community Programs team at the Daniel Driscoll-Neponset Health Center.
- The Case Manager is responsible for developing, implementing and evaluating case management needs and care plans to address psycho-social issues and the case management needs of patients living with HIV/AIDS receiving care at the Daniel Driscoll Neponset Health Center.
- Practices under DPH, HRSA HAB, and BPHC case management standards of care and periodically evaluates and ensures that standards are met.
- Supports food access programs including assisting in operation the weekly food pantry at Daniel Driscoll Neponset Health Center between 7:30 AM and 3:00 PM, coordinating meal delivery and tracking food access support services such as grocery store gift cards.
