Home Care AideWe are looking for a Home Care Aide to join our team! You will be directly working with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health and Mental Health experience is a plus.RESPONSIBILITIESCoverage is needed Monday through Friday (9am to 5:00pm) (11pm to 7am) and every other weekend Per-Diem (As Needed)To assist the client to and from doctor's appointmentsMaintain records of patient care, conditions, progress or problems to report and discuss observations with supervisorEngage patients in exercises or other activitiesGive medication remindersAssist patients by doing laundry, cleaning the home, or assisting with personal care (if needed) and cookingProfessional caregivers go by many names: homemakers, home care aides, home health aides, certified nursing assistants, personal care assistants, direct care workers.
