Job Overview: We are seeking an enthusiastic and skilled Community Registered Nurse (RN) to join our dynamic team! As a Community RN, you will play a crucial role in providing direct care to our clients, ensuring they receive the highest level of medical support and attention. Key Responsibilities: Patient Care: Provide direct nursing care to clients in their homes, including assessments, medication administration, wound care, and other medical treatments. Care Planning: Develop, implement, and regularly update individualized care plans in collaboration with clients, families, and other healthcare professionals. Health Monitoring: Conduct regular health assessments and monitor clients’ conditions, adjusting care plans as necessary to ensure optimal outcomes. Client Education: Educate clients and their families on managing health conditions, medications, and preventative care practices. Collaboration: Work closely with other members of the healthcare team, including physicians, caregivers, and social workers, to coordinate comprehensive care. Documentation: Maintain accurate and timely documentation of all care provided, ensuring compliance with regulatory requirements. Experience in home health, community health, or a similar setting is preferred. Strong clinical skills and the ability to work independently in the field. Compensation: Competitive Salary: Starting at $50 per hour, with opportunities for salary increases based on performance and experience. Mileage Reimbursement: Receive reimbursement for travel expenses incurred while visiting clients.
