Public/Community Health Nurse Practitioner

Community Health Services Nurse Practitioner / Physician Assistant (NP/PA) Position Summary The Community Health Services Nurse Practitioner or Physician Assistant provides comprehensive primary and preventive healthcare services to individuals and families within a community-based setting. This role focuses on improving access to care, managing acute and chronic conditions, promoting health equity, and delivering culturally competent, patient-centered care to diverse and underserved populations. Essential Duties and Responsibilities Provide comprehensive primary care services including health assessments, diagnosis, and treatment of acute and chronic conditionsDeliver preventive care services such as health screenings, immunizations, and wellness educationManage chronic diseases including diabetes, hypertension, asthma, and cardiovascular conditionsProvide care coordination and referrals to specialty, behavioral health, and social support servicesAddress social determinants of health and connect patients with community resourcesPromote health education, disease prevention, and lifestyle modificationProvide care across the lifespan as appropriate to scope of practiceCollaborate with physicians, nurses, care managers, social workers, and community partnersMaintain accurate, timely clinical documentation in the electronic health recordParticipate in population health initiatives, outreach programs, and quality improvement activitiesSupport care transitions and continuity of careComply with all regulatory, accreditation, and professional standards Qualifications Graduate of an accredited Nurse Practitioner or Physician Assistant programCurrent NP or PA certification and state licensureEligibility for credentialing and privilegingDEA registration or eligibilityPrior experience in community health, primary care, or public health preferred Skills and Competencies Strong primary care and clinical assessment skillsKnowledge of community-based healthcare delivery and population healthAbility to work with diverse and underserved populationsExcellent communication, cultural competency, and patient education skillsAbility to work collaboratively within interdisciplinary teamsCommitment to health equity, prevention, and patient-centered care Work Environment Community health centers, outpatient clinics, mobile health units, or public health facilitiesMay include extended hours, outreach activities, or limited call depending on program needs Physical Requirements Ability to sit, stand, and move throughout clinic and community settingsManual dexterity for examinations and clinical proceduresVisual and auditory acuity sufficient for patient assessment and communication
