RN Navigator Medical Imaging - Strategic Insurance Development - Full Time Day

Up To $25,000 Sign On Bonus For Qualified RNs!Summary:The Medical Imaging Nurse Navigator supports continuity of care for patients with incidental findings identified on radiological imaging across the Guthrie health system. This role coordinates navigation, triage, and follow-up care in accordance with established guidelines and provider recommendations. The position serves as a central point of coordination across radiology, clinical providers, and patients to reduce care gaps, improve follow-up compliance, and support timely management of clinically actionable findings. The scope of this role may evolve as imaging technologies, clinical programs, and system priorities mature.Experience:A minimum of five (5) years relevant clinical experience. Demonstrate leadership and autonomy in nursing practice. Preferred experience with care management, navigation, population health, outpatient clinical operations, or diagnostic imaging environments.Education:Licensed Registered Nurse with at least 5 years' experience, preferably in an outpatient/ambulatory setting. Bachelor's degree in nursing or related field required. With an employment agreement, will consider applicants who are actively pursuing their bachelor's degree.Licenses:The Medical Imaging Nurse Navigator must be licensed in both New York and Pennsylvania. The applicant must have a current license as a Professional Registered Nurse in their state of practice prior to position start date. Additional state licensure must be obtained within 6 months of hire. Patient outreach and contact will be limited to those patients living in the state of current licensure until dual licensure is obtained.Essential Functions:Incidental Findings Identification & TriageIdentify patients with incidental findings detected on radiological imaging across care settingsReview imaging reports and provide recommendations, in clinical context, to determine appropriate next steps for follow-up and coordinationApply established criteria to distinguish findings requiring navigation versus routine follow-upPatient Navigation & CoordinationContact patients to review imaging results and recommend follow-up as communicated by the ordering or receiving provider, and outline next steps in careCoordinate follow-up imaging, specialty referrals, or provider appointments as appropriateAddress barriers to care such as scheduling access, insurance concerns, or patient understandingSupport patient engagement and adherence to follow-up recommendationsClinical & Operational CollaborationCollaborate with radiologists, ordering providers, receiving providers, and operational teamsSupport prioritization of time-sensitive or clinically significant findingsMonitor completion of recommended follow-up, as defined by program workflows, and support outreach for missed or delayed care as appropriateAdapt workflows as programs expand across imaging modalities and clinical specialtiesOther Duties:Travel for this position may be required.The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements as to his/her specific needs, and to provide the care needed as described in the appropriate policies and procedures.It is understood that this description is not intended to be all inclusive, and that other duties may be assigned as necessary in the performance of this position.#LI-RB1
