Facility:Child Health PavilionDepartment:Social Determinants of HealthSchedule:Full timeHours:40Job Details:The Economic Support Navigator supports the hospital's Population Health and Care Coordination teams in alignment with the work of Partners for Kids to address social determinants of health (SDOH) and improve family economic stability. This role assists patients and families with accessing public benefits; specifically Supplemental Security Income (SSI) enrollment and Medicaid redetermination and strengthens the hospital's ability to support economic stability through community and governmental partnerships.Required QualificationsEducation:GED or High School diplomaExperience1 year or more experience working with public benefits, such as SSI or Medicaid.Skills and Competencies:Strong communication, organizational, and problem-solving skills.Ability to work effectively with diverse populations and multidisciplinary teams.Experience with MS Office products.Preferred QualificationsAssociate in social work, public health, community health, or a related field preferred3-5 years' experience working in population health, care coordination, social services, or community resource navigation.Familiarity with SDOH frameworks.Direct experience collaborating with SSA, JFS, or similar agencies.Department Specific Job Details:full time, 40 hoursMonday - Friday, 8:30 AM - 5:00 PMserving the Center for Community Health at the Connor Child Health Pavilion
