Primary Care Physician – In-Home Services (NJ)

Salary Range: $220k–$250k (varies depending on years of experience) This organization is redefining how primary care is delivered to older adults with complex medical needs by bringing high-quality, relationship-based care directly into the home. Physicians work closely with a dedicated clinical support team to deliver comprehensive, patient-centered care to a defined panel of patients. What Makes This Role Unique Visits are conducted with a MA, allowing you to focus on clinical care No weekend visits Clear pathways for growth and advancement within the organization Key Responsibilities Deliver longitudinal primary care to patients in their homes Conduct in-home medical visits focused on symptom management, chronic disease care, and preventive services Partner with an interdisciplinary team including MAs, nurses, social workers, and other clinicians to execute individualized care plans Identify drivers of hospitalization risk and proactively address them through care planning Lead and participate in interdisciplinary care discussions and team meetings Coordinate specialty care, hospital transitions, post-acute services, and community resources as needed Complete clinical documentation accurately and efficiently using a proprietary, technology-forward platform Contribute to ongoing program improvement and evolving care models Qualifications MD or DO required Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine Active New Jersey medical license Minimum 5 years of clinical experience Prior experience with home-based care strongly preferred Strong communication skills and cultural sensitivity when working with diverse populations Additional Highlights Structured schedules with no weekend responsibilities Highly supportive clinical leadership and team-based care model Technology-forward environment that minimizes administrative burden Long-term career growth opportunities!
