Nurse Practitioner, Primary Care/Family Practice

Organization: SLH-Sutter Lakeside Hospital Position Overview: Assesses, diagnoses, plans and implements the medical care of patients in collaboration with a physician. Functions within the full scope of licensure; accountable for the assessment, coordination delivery and evaluation of nursing care, including direct patient care, patient/family education and transitions of care. Delivers holistic and individualized care to all patients in assigned area. Develops, implements, manages an optimal interdisciplinary plan of care that incorporates psychological, sociocultural, spiritual, economic, and life-style factors. Fosters and maintains collaborative relationships between patients, their family/support group, physicians, and other healthcare providers through timely and effective communications. Adheres to entity polices, industry standards, best practices, and applicable laws/regulations and codes to promote a quality, highly reliable patient experience. Engages in continuous growth and development in professional advanced nursing practice. Follows ambulatory care and inpatient standardized procedures, protocols, policy and procedures regulated by the State of California Nurse Practice Act. Job Description : ASSESSMENT AND PLANNING OF PATIENT CARE Conducts comprehensive patient and family assessments, including physiological, emotional, and spiritual needs. Collaborates with physicians and interdisciplinary team members to develop and adjust patient-centered care plans. Communicates effectively to ensure continuity and coordination of care. Partners with peers, providers, and leadership to improve patient outcomes and care quality. Supports workflow optimization to enhance patient care delivery. Communicates care plans across teams and departments. Facilitates patient transitions to community providers when appropriate. PROVISION OF PATIENT CARE Delivers high-quality, compassionate, and cost-effective care aligned with organizational policies and evidence-based practices. Maintains a safe, therapeutic environment for patients and their families. Delegates tasks appropriately based on team members’ scope, licensure, and competency. Provides clear, patient-centered education using teach-back methods to ensure understanding. Practices mindful, individualized care with a strong focus on patient safety. Ensures compliance with all organizational, local, state, and federal regulations. Safeguards patient confidentiality and protects all protected health information (PHI). Stays current with advancements, best practices, and trends in clinical care and specialty areas. Participates in and supports process improvement initiatives at all organizational levels. EVALUATION OF PATIENT CARE Monitors interventions, evaluates patient responses, and adjusts care plans accordingly. Provides ongoing patient and family education throughout the continuum of care. Reassesses and documents patient progress and outcomes. Reports safety events or near-misses through proper reporting systems. Participates in the analysis of safety events to prevent recurrence. Maintains accurate and timely documentation in the electronic health record (EHR). Develops and implements comprehensive discharge and transition education plans involving patients and families. EDUCATION Graduate of an accredited school of nursing Master’s Degree in Nursing LICENSURES & CERTIFICATIONS Nurse Practitioner (NP), State of California Nurse Practitioner Furnishing License (NP-F) Registered Nurse (RN), State of California Controlled Substance Registration (DEA) (required if prescribing) Basic Life Support (BLS) – Healthcare Provider Additional certifications as required by department TYPICAL EXPERIENCE Ideally minimum of 5 years of recent Relevant clinical experience Job Shift: Days Schedule: Full Time Days of the Week: Monday - Friday Weekend Requirements: Occasionally Benefits: Yes Unions: No Position Status: Exempt Weekly Hours: 40 Employee Status: Regular Sutter Health is an equal opportunity employer EOE/M/F/Disability/Veterans. Pay Range is $175,489.60 to $254,446.40 / annual salary The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs. Base pay is only one component of Sutter Health’s comprehensive total rewards program. Eligible positions also include a comprehensive benefits package.
